So have been enjoying my tank and I think I just need one more fish for my 180gal

I was thinking of adding a powder blue tang but my brother says they are all a bunch of a-holes and wanted to get feedback from those who have them :)

My current stock:

4" Blue Tang
3" Yellow Tang
3" Flame Angel
Yellow Wrasse
Red Coris Wrasse
Hoeven's Wrasse
Flame Hawk
2 Ocellaris Clownfish
2 Firefish
2 Purple Firefish
Diamond Watchman Goby

Should I pull the trigger on a 4" to 4 1/2" Powder Blue Tang that has been medicated ad eating well?

I think he'll definitely have problems with ur established tangs, but only one way to find out!
I like the way you think

My plan was to wait to add him to the display until lights out and everyone starts going to sleep and get him a little bigger :)

PBT are very territorial. Adding him last may help with the aggression issue. Is your blue tang the boss of the tank now? If so, I think it will meet it nemesis.

Medicated with what? And how long? PBT are one of the hardest tangs. Some have lost most of their fish because any stress can cause an ick break.

My yellow is the boss, the blue really doesn't bug anyone but no one messes with him either

I am happy with my tank but feel one more tang will pop with color and make it perfect for me

will the "change" ever calm again?

Not sure, I have to say I'm in love with tangs. Sohal, achilles, chevron, powders!!!!! I love them all, That being said all the chaos they cause is so stressful. I'm happier now with less tangs. If your looking to add a final piece maybe something special like a white tail tang beutiful but not as angry lol . That balance is just so hard to get sometimes. Just my 2 cents
