The zoa is sitting at the bottom of the tank where 1 of 3 polyps are opened. That's why I'm not sure if there's any pest of anything was annoying it.
When I tried to see if there's any pest, that blister was the only I can see. I'll take another look if it still...

Since I haven't make an effort to bump the kH yet, I am wondering if I should dose soda ash given the numbers (I know marine buffer have Boron)...

I am using Red Sea Coral Pro Salt. For Salinity, I have two refractometer and I use them to cross reference the salinity... Since it's been so hot these days with the evaporation, that might be an issue (I top it off every night though).
