Is 5g too small for a QT? I won't be keeping big fishes because my DT will only be 40g.

Also I know constant water flow is a must to keep the water from becoming stagnant but what about temp? If there's no fish in there at the time do I need to keep the water temp within range? I have live sand and a few small rocks in there.

Temperature will make a huge difference in the health of the fish. Too cold or hot and they'll stress out leaving them more vulnerable to diseases

Yea I'm going to pick up a heater for it but my question is, if there's no fish in there at the time do I need to maintain the temp?

maintain everything..you need bacteria to culture...also I would get rid of your rocks and sand too. especially if you'll be medicating them.
