Ich- Lieutenant Tang Gone

Ok so I notice ich/ick on my blue tang and lieutenant tang out of all my fish (12) in my 40 gal one day. Little worried I go to Lfs and grab garlic and kick-ich. I take out charcoal, and turn off skimmer. Feed tank 1 block mysis shrimp thawed and with 2 drops of garlic and a 2"x3" piece of seaweed with 2 drops of garlic rubbed in. Dose tank for 4oz of kick-ich(2oz per 25gallons). So next day I wake up and all gone! Stoked but aware it's probably still in substrate I keep dosing and feeding plenty of greens and garlic(not exceedingly). And 2-3 days later it's back. I keep doing doses every other day as bottle directed. Then next day Blue tang Back to health but lieutenant tang isn't doing so well. Next day he is doing horrible and breathing heavy with white spots everywhere on him and flakey skin. The following morning it dies. Everything else seems to not have any physical signs of ich at the moment but not sure what to do.

Well one thing that could be causing stress is the 12 fish in a 40 gallon tank. Also watch and see if any of the fish are bullying the Tangs.

agreed...2 tangs along with 10 other fish in a 40 gallon is a little much...monitor your water quality. Ich is a sign of stressed fish. You should lessen the bioload.

sorry but the guy's are correct. Not sure what your other 10 fish are but 40 gallons isn't large enough for Tangs. I have two Tangs is an 50 gallon with two other small fish, once they get big enough to hang with the big boy's they get moved to my 270 gallon.
Some fish have recommended tank sizes because of how big they get, 40 to so small for any tang after 6 months to a year. depending on the size of the other 10 fish you have i large bio load. One inch per gallon for small fish. large fish need a large home.
Sounds like you did the right thing to get them healthy but i would say just the stress from a small tank with many fish is your down fall.

Ok so i use ph buffer to maintain 8.3 ph. Also I am aware I am taxing the heck out of my filtration but the other fish are small, 2 (1 1/2")small Chromis, 2 (3") bar gobies,
(2 1/2"-3") blue tang, (3") yellow tang, (3 1/2") ocellaris, (2") six line wrasse, and (3") diamond goby. They all get along fine and no problems. I'm now down to 9 fish to help empty tank. Ich is clear so far, no physicl signs of it now so maybe lightening the load helped.
