That's what I'm thinking.

Is there something fish-behavior wise that could clue in that that's what happened? My very last one was doing really well, and then started frantically swimming, bumping into the glass one morning and tipping over for lack of better words and died at the end of the day

If the stray voltage is bad you would feel it just by sticking ur hand in the water, otherwise u might need a meter. I think some of the local reef clubs have them and share around with club members if I'm not mistaken

Hmmm I'll definitely be on the lookout for something that can test it out then. Would that also mean that my cleaner's immune to whatever stray voltage is in there?

How are you running your QT? What is the set up, size, parameters?

Are you running copper or hypo salinity?
I ran a small spare tank I had lying around with just a heater, sponge filter and pvc fittings. I set it up just for observation, so I didn't dose anything into it, but I did have it cycle before I added fish to it.

Where did your live rock come from? You may have a hitchhiker that is killing the fish. The main things to look for are Eunicidae type worms, they look like large Centipede and can eat coral and kill fish. Large crabs that hide in rocks, or some type of Mantis shrimp. A old piece of panty hose with a rock and some food in it tie wrapped together in a easy spot to see and a red flash light in the middle of the night. Scary creatures can come out of live rock! If your rock was live something to check, your parameters seem fine and your coral would die before your fish if there was something so out of whack to kill a clown. Sorry to hear your troubles and I would try and pick up a pair of clowns from somebody on the boards over some babies.

I'll keep a lookout for hitchhikers, I got my live rock from Ellis Aquatics

Like filter media? Right now, just a small filter sponge I rinse every week and a big blob of chaeto
I would suggest adding some carbon, also, I run chemi pure, purigen, phosban, and GFO. All of those help eliminate poor water quality, also try replacing your filter sponge instead of rinsing it out, most LFS will sell cubic feet of filter pads that you can easily cut to size.

Ok, I'll look into the filter pads, I just needa small sliver of it, so I'm sure a pack of those will last me for a while.

Other than all the testable stuff, how does media affect water quality? I used to run carbon on my tank, but since I've switched it out, everything's been testing a little better and consistently

You can do some simple google searches of the medias I've mentioned, I would never set up a tank with out those 4 that I mentioned.
