Lost a fish this morning, not sure why

My 4" yellow tang, about 3.5 years since I got him was dead on the bottom of the tank this morning. Not sure, but looking for ideas. He seems to have started losing some color, not a lot a few days ago, and was not as aggressive eating as normal. Since he was a bit consumed by the CUC I can't see any physical signs. Two of, but not all of, the LPS (torches) are not happy. However the rest of tank is looking good, the 6 RBTA look awesome and are growing out of control, other torches, hammers, frogspawns also look fantastic. I know, conflicting info.

Temp pretty stable at 78.1-78.5 according to my RKE.
Am:0
NO3: about 10-12 (normal, can never get it below that).
SG: 1.022 (a touch low).

Tank is 400g total water volume (approx.), we change 20-30gallons per week.

No known predators, foreign bodies, etc. No kids or people who could dump stuff in the tank. No visible ich or any other type of infection on other fish. Other fish include 4 other tangs (3-4"), 3 angels (2-3"), 3 cardinals, 4 clowns, royal gramma, 3 gobies, leopard wrasse (low key), 2 mandarins + 1 clown in sump.

Same bucket of salt for the last 4-5 changes

RODI is self made, filters are about 3 months old.

Sorry sucks to have a sudden loss. Especially of a fish that youv had for so long. You can look for answers but sometimes there aren't any. It's frustrating but Sometimes it's just their time to go:(

Similar thing happened to me with one of my tangs. But I later found out my nitrates were up the wazooo... sorry for your loss.
