My Bird wrasse has weird scratches on his face

My bird wrasse has recently been getting these scratches on his face that have been getting white and and it just looks like there is something on him. My first thought was that it was a parasite so my course of action was to prepare a freshwater dip. Immediately cater the dip I took the photo that was attached. His scratches were no longer white and overall everything looked better. Although he was very stressed out and my serpant star very ambitiously tried to take a bite he eventually recovered and was back to his old self. But within a day or so the white on the scratches started to return and he has since stopped eating and I don't know what else to do please help !
