Need help Id what is on my hippo tang

So today I found my blue hippo tang laying on the sand and I took it out and can't figure out what happened to it. If someone could please tell what it is and why it happened so I know how to prevent it for the future, I greatly appreciate it. Thanks in advanced

Looks like ich. Maybe even black ich

It's just really weird because I've had this tang for 10 months and he's always eaten and never acted weird to suspect any kind of disease on him.

+1 ich possible black ich. Maybe a new frag, live rock, hermit crab etc that came from an infected tank.
